Patient has had chronic right-sided hearing loss for four years, and tinnitus for three years associated with mild vertiginous symptoms. He has noticed right-sided facial paraesthesia over the last year and a dry right eye.

Moderate COPD, FEV1/FVC 65% predicted

Aspirin, urticarial and angioedema

FHx

Father died of laryngeal cancer; mother is alive and in high level care nursing home with end stage renal disease. No siblings.

SHx

Patient lives with wife and three children in Armidale. He works as a librarian in the town library. He quit smoking four years ago when he was first diagnosed with COPD; he has a 20 pack year history. He does not drink alcohol and follows a strict vegan diet. He is a member of a local swim squad that meets three times a week.
