Patient experienced acute onset back pain six weeks ago after lifting and swinging her four-year-old son. The pain is felt in the back, but worse down the right leg. She has noticed some weakness in her right leg and paraesthesia on the sole of her foot. |

Mother has T2DM. Father has had a stroke, which left him with severe deficits. Siblings and children are all well. |

Patient lives with her husband and their three children (10, 6, 4) in Armidale. She works as an accountant and helps her husband run a small surveying business. She is a non-smoker and only drinks alcohol socially. Her diet is poor and physical activity limited which she attributes to her busy lifestyle. |
