Mr Tucker is a 56-year-old man who has presented to the Armidale Hospital Emergency Department with a two-day history of obstipation, worsening abdominal pain and nausea. Today be developed vomiting and his pain has again worsened so presented to ED.

The pain is generalised and central in the abdomen and is colicky in nature. He has been noticing increasing pain in the right lower quadrant. There is a lump in his right groin which would often disappear when he laid flat, but this had not gone away over the last few days. It is becoming quite tender to touch. This groin mass had been slowly increasing in size over the last few years but never caused him any issues. If he had been on his feet all day, it would sometimes ache in the afternoon, but this was rare.

Mr Tucker is quite concerned as he has never experienced anything like this before.

Mr Tucker is a real-estate agent who lives in Uralla with his wife and son. They have 2 adult daughters that have moved out of home and 1 Son who is currently completing his HSC and lives at home.

He has a smoking history of 40 pack years and has about 10 standard drinks a week. He is able to walk up 2 flights of stairs, but becomes extremely out of breath.
